Release of any academic records require the signature of the student and should be mailed to the address shown above or faxed to 901.448.7772. 

All web page submissions require UTHSC UserID and Password via Student Information System (SIS) applicable to enrolled students only.  Requests are processed within 2 to 3 working days.
