Confirmation of Attendance

In order to avoid cancellation of a schedule, all students must complete the STUDENT FEE ASSESSMENT FORM(Fee Sheet). This form must be signed, dated, and received in the Bursar’s Office on or before the published due date of each term. Transfer of financial aid, departmental or third-party funds will not be released to the account of a qualified student until the Fee Sheet is received. Confirmation is set when a student pays the full or minimum amount due on his/her account by the due date. All holds/prior balances must be cleared prior to your registering.

• Write a check payable to University of Tennessee OR sign fee sheet 
  authorizing the University to pay any outstanding fees with your loan 
  checks.  If your loan or scholarship checks do not cover your fees, you
  must pay the difference when you register.  No Credit Cards Accepted.
  
  
• If a portion of your fees are to be paid by someone else (Army, Emp.
  Discount, etc.) provide appropriate supporting documents.

Installment payment plan

If you wish to go on the installment plan please read the information on the Installment Application Form
provided on the Bursar's web site. Please print the form and submit with your fee sheet.

Fee deferment

If you received, signed and returned your awards letter to the Financial Aid Office, please sign the fee
sheet and return it to the Bursar's Office and your fees will be deferred. If you did not receive an
awards letter and are expecting financial aid, you must contact the Financial Aid Office at (901) 448-5568
or e-mail fao@uthsc.edu. The Financial Aid Office is located at 910 Madison, Suite 520.

Insurance

If you have your own insurance, please provide proof to the Student Life RM 309, SAC or contact them at
448-4860. Otherwise you will automatically be enrolled in the plan with a single coverage cost. A refund can only be
processed through Gerald Holland Insurance at (662)895-5528 or (888) 393-9500. Request must be made within
