Obstetrical Care:
The Division of Maternal-Fetal Medicine is responsible for the high-risk obstetric care at the Regional Medical Center’s BirthPlace. The Center has a large maternal referral base from 80 counties and five states. With approximately 5000 deliveries a year, many patients have significant high risk maternal and/or fetal problems. The labor and delivery unit at the Regional Medical Center has 15 LDR rooms as well as the capacity for ICU care. The high-risk service is maintained providing focused care to complicated antepartum and postpartum patients and is available for consultation on patients in Labor and Delivery Unit 24 hours a day.
Outpatient facilities are located in the adjoining 880 Madison Office Building. A State-of-the-art Antepartum Fetal Evaluation Unit is located in the Medplex Ambulatory Clinic (880 Madison Office Building). Consultation services for high risk patients from referring physicians and total care for the most complicated patients; there are more than 8000 High-Risk visits a year. The unit has dedicated nurses, sonographers, dieticians, pharmDs and psychologists. High level ultrasound with 4-D ultrasound capabilities, fetal echocardiography, first-trimester screening, prenatal diagnosis (including amniocentesis, chorionic villus sampling and percutaneous umbilical artery sampling) and genetic counseling are all routinely available. The unit performs approximately 7500 ultrasound examinations a year. An additional outpatient site at 7945 Wolf River Blvd Germantown, TN also exists as a site for referrals for ultrasound and prenatal genetic counseling.
The Division hosts regularly-scheduled conferences in order to discuss care of the complicated patients. Pediatric surgeons and neonatologists attend to discuss the care for patients carrying fetuses with anomalies. The Division also publishes protocols as guidelines for care of certain diseases. The Division also maintains an extensive database and is responsible for outreach education to our regional community partners.
Education:
Residents play an integral role in the division’s care for the high-risk patients. The MFM rotation team consists of PGY1, PGY2, and PGY3 level residents. The PGY3 takes ultimate responsibility for the hospitalized patients and organizes the team to fulfill the duties. Residents also attend clinic in order to learn to care for high risk patients. Typical patient pathologies cared for by residents include hypertensive disorders, diabetes, hemoglobinopathies, asthma, thrombophilias and collagen vascular diseases. Goals and Objectives for resident education follow CREOG and ACGME guidelines; education and competency is evaluated.
In addition to conferences with neonatology and pediatric surgeons mentioned above, a Tracing Conference is held weekly where residents and faculty members review fetal heart rate tracings and discuss management.
Medical students participate in care of high risk patients as a member of the team both on the wards and in clinic during their third-year clerkship on Ob-Gyn. In addition, elective rotations in Ultrasound and High Risk Medicine are also available for fourth-year students.
Research:
The Division of Maternal-Fetal Medicine is actively conducting studies in both clinical and bench research. Ongoing research activities are currently funded by the Center of Disease Control, the March of Dimes, and other national organizations facilitating national and international collaborative efforts. In addition, pharmaceutical industry sponsored research focusing on a variety of maternal-fetal issues is currently underway studying investigational drugs and devices. The Division is also engaged in collaborative efforts with the Division of Neonatology compiling a database of maternal-fetal records, and the State of Tennessee to provide outreach through collaboration. The Division is also working concurrently on research projects with investigators from the Department of Cardiology, the Department of Pediatrics, and College of Nursing, College of Anesthesia, and the Fogelman College of Business.
In addition, the maternal fetal faculty members are active sponsors and mentors in resident research projects. They provide guidance regarding regulatory submissions, study design, and planning and implementation of research projects. Many of these projects are presented at regional and national meetings and ultimately published.
